Driver Qualifications
According to Federal Motor Carrier Safety Administration regulations, each motor carrier shall maintain a driver qualification file for each driver it employs. The qualification file for a driver must include certain items as outlined in the FMCSA regulations (Part 391.51: General requirements for driver qualification files).

The DQF:
- The driver qualification file must be maintained as long as the driver is employed and 3 years thereafter.
- The driver qualification file must include:
- An Employment Application.
- A Copy of the Driver's Medical Certificate.
- The Road Test Certificate or Equivalent.
- The State Driving Record Inquiry.
- Inquiries to Previous Employers.
- Driver's Annual Certification of Violations.
- Annual State DMV Violations Inquiry.
- Annual Employer Review of Driver's Record.
The SPH File:
- Notices Regarding Late and Missing Logs.
- A driver violation listing that tells you which drivers are not in compliance and what requirements they are not in compliance with.
- Management Summaries showing both company and individual driver violation severity levels.
- Dated Hours of service recaps.
- The following notifications for you to send to your drivers:
- Violation Letters informing them of any non-compliance issues.
- Congratulatory Letters to let drivers know that they are in compliance with the regulations and violation free.
DOT Drug Testing:
- Drug screen test of new employees.
- Random drug testing of current employees.
- Post accident drug testing.
- Maintenance of record of drug testing in DQF file.
DOT Medical Exams:
- DOT physical exam of new drivers (when required).
- Review of DQF files to make sure DOT physical exams are current.
